INSURANCE INFORMATION

We are an out of network provider. As a courtesy, we will submit charges to your insurance company if you have Horizon, Blue Cross, Blue Shield, Aetna, or United Healthcare.  If you have any other insurance, you will need to submit the charges yourself.  We will provide you with any documentation you may need to do this.

Before beginning treatment, it is important for you to know what is and is not covered by your insurance. 

The following are questions to ask your insurance company 

1.      Do I have out of network benefits?

2.      If so, what is my deductible, and what is covered after I meet my deductible?

3.      Is there a maximum charge allowed per session?

4.      Do I require pre-authorization?

5.      How many PT sessions am I allowed per year? 

6. Are physical therapy evaluations covered? (Some insurances cover physical therapy treatment, but not the evaluations.)
